NOTICE OF PRIVACY PRACTICES

Axon Medical Group PC d/b/a AuraAI

Effective Date: November 1, 2025

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE READ IT CAREFULLY.

OUR PLEDGE TO YOU

At Axon Medical Group PC (doing business as AuraAI), we believe that trust is the foundation of the patient-physician relationship. We understand that your medical information is deeply personal, and we are committed to protecting it with the highest standards of security and confidentiality.

While we utilize state-of-the-art Artificial Intelligence (AI) to provide world-class care, we apply those same rigorous standards to your privacy. We do not sell your personal health information. Every tool we use, and every partner we work with, is selected based on their ability to meet strict federal HIPAA security standards.

This Notice describes our legal duties and privacy practices. It explains how we safeguard your information while ensuring you receive the best possible medical care.

HOW WE PROTECT AND USE YOUR INFORMATION

To provide you with comprehensive care, we must use and share your health information within a secure "circle of care." We treat your data with the utmost respect. Below are the specific ways we use your information to help you.

1. Delivering Your Care (Treatment)

We use your information to diagnose and treat you. We may share information with other doctors, nurses, or trusted healthcare providers who are directly involved in your well-being.

  • Reassurance: We only share what is necessary for your treatment. For example, if we refer you to a specialist, we share your relevant records so you don’t have to repeat your history, ensuring a seamless healthcare experience.

2. Innovation with Integrity (Our Use of AI)

AuraAI is a forward-thinking practice. We use HIPAA-compliant Artificial Intelligence to ensure your care is accurate, thorough, and focused on you.

  • Safety & Security: We only use AI software that is fully encrypted and HIPAA compliant. These vendors are legally bound by "Business Associate Agreements" to protect your data just as strictly as we do.

  • How it helps you:

    • Pre-Visit Preparation: An AI agent may interview you before your visit to gather symptoms. This ensures your doctor is fully briefed on your story before they walk into the room.

    • Safety Net: We use AI to review your past medical records to catch trends, risks, or history that might otherwise be missed.

    • More Eye Contact, Less Typing: During your visit, we use secure "ambient listening" AI to transcribe the medical note. This allows your doctor to look at you and listen, rather than staring at a computer screen.

  • Human Oversight: Technology never replaces your doctor. All AI-generated summaries and notes are reviewed, verified, and finalized by your human healthcare provider.

3. Managing Your Account (Payment)

We use your information to bill and collect payment from you, your insurance company, or a third party.

  • Reassurance: We only share the specific medical codes and information required by your insurance plan to approve and pay for your care.

4. Improving Our Practice (Health Care Operations)

We use health information to run our practice efficiently and ensure all our patients receive quality care. This includes training staff, quality improvement assessments, and reviewing clinical outcomes.

STRICTLY LIMITED DISCLOSURES

Federal law requires us to list the following scenarios where we may be required to share information. While these situations are rare for most patients, we want to be transparent about our legal obligations. In all cases, we limit disclosure to the minimum necessary requirements of the law.

  • Public Health & Safety: We may disclose information to public health authorities to prevent serious disease, report adverse reactions to medications (FDA), or prevent a serious threat to health or safety.

  • Required by Law: We will disclose information when explicitly required by federal, state, or local law (e.g., reporting abuse or neglect to protect vulnerable individuals).

  • Legal Proceedings: We may disclose information in response to a court order or a valid subpoena, provided that efforts have been made to notify you or secure a protective order.

  • Law Enforcement: We may release information if asked by law enforcement officials regarding a crime or suspect, subject to strict legal limits.

  • Deceased Patients: We may release information to a coroner or funeral director to allow them to carry out their duties.

  • Specialized Government Functions: In rare cases involving national security, military service, or protective services for the President, we may be required to release information.

  • Workers’ Compensation: We may release information for workers’ compensation claims as authorized by law.

YOUR RIGHTS: YOU ARE IN CONTROL

You have specific rights regarding your health information. We are here to help you exercise them.

  1. Right to Inspect and Copy: You can review and get a copy of your medical and billing records.

  2. Right to Amend: If you feel your record is incorrect, you may ask us to amend it.

  3. Right to Request Restrictions: You can ask us not to use or share certain information. While we are not always required to agree, we must agree if you ask us not to disclose information to your health plan for a service you paid for out-of-pocket in full.

  4. Right to Confidential Communications: You can ask us to contact you in a specific way (e.g., only on your cell phone or by mail to a different address).

  5. Right to a Paper Copy: You may ask for a paper copy of this notice at any time.

  6. Right to a History of Disclosures: You may ask for a list of times we shared your information for reasons other than treatment, payment, or operations.

CHANGES TO THIS NOTICE

We reserve the right to change this notice. If we make material changes, we will post the revised notice on our website and make it available at our office. The changes will apply to information we already have as well as new information.
